RELEASE OF LIABILITY, WAIVER OF CLAIMS AND ASSUMPTION OF RISKS. BY SIGNING THIS DOCUMENT, YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. PLEASE READ CAREFULLY.
ASSUMPTION OF RISKS
I understand that martial arts training and the use of the facilities of PRODIGY TRAINING CENTER (the "GYM") involves some risks, dangers and hazards including, but not limited to, physical contact with
other students and instructors (e.g. punching, kicking, throwing, joint manipulation and submission holds), accidental collisions with other persons or objects, failure of equipment, failure to perform within one's own ability or within designated areas, failure to follow instruction, human error and negligence of other persons. I freely accept and fully assume all such risks, dangers and hazards and the possibility of personal injury, death, property damage and loss resulting therefrom. I am aware
that the gym does not carry accident insurance on my behalf.
RELEASE OF LIABILITY AND WAIVER OF CLAIMS
In consideration of the gym permitting me to use its facilities and to receive martial arts training, and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I hereby
agree as follows:
1. TO WAIVE ANY AND ALL CLAIMS that I may have against the gym, its shareholders, directors,
officers, employees, contractors, agents and representatives, including anyone authorized to provide
martial arts instruction (all of whom are herein after collectively referred to as "the Releasees"):

TO RELEASE THE RELEASEES from any and all liability for any loss, damage, injury or expenses that I may suffer, or that my next of kin may suffer, as a result of my use of the gym’s facilities, participation in martial arts training, or participation in any activities organized, operated or sanctioned by the gym, wheresoever located, due to any cause whatsoever, INCLUDING NEGLIGENCE, BREACH OF CONTRACT, OR BREACH OF ANY STATUTORY OR OTHER DUTY OF CARE, ON THE PART OF THE RELEASEES:
3. TO HOLD HARMLESS AND INDEMNIFY THE RELEASEES from any and all liabilities for any property damage or personal injury to any third party, resulting from my use of the gym’s facilities, participation in training or other activities as aforesaid
4. This Agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns and representatives in the event of my death or incapacity.
I have read and understand this Agreement and I am aware that by signing it, I am waiving certain legal rights which I or my heirs, next of kin, executors, administrators, assigns and representatives may have against the Releasees
